چکیده

Background: Abdominal aortic aneurysm (AAA) carries high morbidity and mortality, while adverse events (AE) after endovascular aneurysm repair (EVAR) remain a major challenge. Accurate risk stratification is essential. Radiomics, by extracting quantitative imaging features, offers a noninvasive approach that may improve prediction of AAA outcomes and EVAR-related AE beyond conventional imaging. Main body: A search was conducted across Medline, Scopus, Embase, and Web of Science databases for articles published up to February 2024 to evaluate the application of radiomics in predicting the outcomes of AAA and diagnosing or predicting the AE related to EVAR. Inclusion criteria concentrated on observational studies that utilized a radiomics model based on the radiomic features extracted from imaging modalities to predict AAA outcomes and predict or diagnose EVAR-related AE. Eight studies involving 1729 observations met the inclusion criteria out of the 371 records yielded from the databases. Radiomics achieved a high area under the receiver operating characteristic (ROC) curve (AUROC) for predicting EVAR-related AE in three studies (up to 0.95), and it showed predictive value for AAA growth in three studies (AUROC 0.79-0.93) and rupture or repair in one study (AUROC 0.75). One study highlighted the diagnostic value of radiomics in post-EVAR endoleak detection using unenhanced computed tomography images (AUROC 0.91). Conclusion: Radiomics showed promising but preliminary results in predicting AAA outcomes and EVAR-related AE, providing a noninvasive tool for risk assessment. However, further research with larger cohorts and external validation of the radiomics models is essential to confirm its clinical applicability.

چکیده

Background and aims: Hemorrhoids are common vascular structures that can become symptomatic when prolapsed, representing the most prevalent benign anorectal condition worldwide. Despite affecting up to 11% of the population with symptoms including pain, bleeding, and discomfort, hemorrhoids remain underdiagnosed and often inappropriately managed. This systematic review aimed to evaluate the efficacy, clinical outcomes, and morbidity of endovascular artery embolization in hemorrhoids. Method: Up to August 2022, PubMed, Scopus, Web of Science, and Embase were searched comprehensively. All studies had eligible criteria included, except review articles, articles published in a language other than English, and case reports or series of cases. Two independent reviewers performed a literature review and data extraction. Preferred Reporting Items for Systematic Reviews and Meta-Analysis (PRISMA) 2020 checklist was used for quality assessment. Results: The systematic review included 15 studies. The patient's age ranged from 40.5 +/- 9.51 to 72.2 +/- 10.9. Eleven studies mentioned the sample size as gender-specific, with 57.89% (n = 187/323) male patients. The embolic site involved in embolization, according to the frequency, was 424 patients for the superior rectal artery in all studies, and coils and micro coils were utilized as embolic material in most cases. No major complication or death was noted in 12 studies. The most frequent complications were postoperative pain and tenesmus, bleeding, ulceration and fibrotic scars, and low-grade fever. In 11.79% (n = 27/229) of patients re-embolization and after-treatment surgery were due to surgical failure and symptom recurrence, such as bleeding. Conclusion: Rectal artery embolization using microcoils, embolic particles, or a combination is a new method for managing internal hemorrhoids with proper clinical efficacy without significant complications. QoL score has improved after emborrhoid, and most cases had a lower bleeding score.

چکیده

Purpose: Cardiac rhabdomyoma, the most common fetal cardiac tumor, is frequently associated with tuberous sclerosis complex (TSC). This study examined pre- and postnatal factors affecting mortality in fetal cardiac rhabdomyoma (FCR) and explored links between TSC and clinical features. Methods: A systematic review was conducted in July 2023 using Medline, Web of Science, Embase, and Cochrane Library to identify reported FCR cases. Analyses were performed on two sample groups (A and B). Results: Neonatal mortality was significantly associated with hydrops (p < 0.001), tumor progression (p < 0.001), fetal heart failure (p < 0.001), and fetal growth restriction (FGR) (p = 0.021). Hydrops was the strongest predictor of mortality (Sample A: RR 17.2, p = 0.027; Sample B: RR 12.2, p = 0.006). Among 683 cases, 73% were diagnosed with TSC. Multiple tumors increased the likelihood of TSC (Sample A: RR 4.8, p < 0.0001; Sample B: RR 3, p = 0.015). In postnatal factors, preterm delivery (p < 0.001), cardiac arrhythmia (p = 0.001), and heart failure (p = 0.001) were also linked to mortality. Conclusions: Hydrops is the strongest mortality predictor in FCR. Multiple tumors are associated with TSC. In prenatal factors, tumor progression, fetal heart failure, and FGR were associated with increased mortality.

چکیده

[Assembled from verified full-text sections; no standalone abstract block was retrievable on the publisher page] This prospective single-arm study evaluated pain trajectories after uterine artery embolization in 34 patients. A prophylactic 100 µg intra-arterial nitroglycerin dose was given per uterine artery before polyvinyl alcohol embolization, followed by 20 mg lidocaine per artery after the embolization endpoint. Mean visual analogue scale pain scores were 2.77 ± 2.03 at 0 hours, 5.38 ± 2.81 at 1 hour, 5.59 ± 2.28 at 2 hours, 6.24 ± 1.67 at 3 hours (peak), 4.12 ± 1.68 at 6 hours, 3.27 ± 1.48 at 12 hours, and 2.69 ± 1.72 at 24 hours. Mean pain correlated with the largest fibroid volume (ρ = 0.51, 95% CI 0.10–0.77, p = 0.02). The authors conclude that the largest fibroid volume is associated with greater post-embolization pain, that nitroglycerin given before embolization allowed subsequent lidocaine administration without observed vasospasm, and that larger randomized trials are needed.

چکیده

Background: Non-invasive lobular neoplasia (NLN) encompasses a range of lobular breast lesions that may precede invasive breast cancer. Microcalcifications detected through mammography play a crucial role in evaluating breast lesions and are often associated with NLN. This study focuses on the prevalence and significance of microcalcifications in NLN, noting that they can be the sole radiographic finding in many cases. While mammography is highly sensitive for detecting microcalcifications, it has limitations in diagnosing NLN, as some lesions may not show up on scans. Advanced imaging techniques like magnetic resonance imaging (MRI) offer improved diagnostic accuracy, particularly in dense breast tissue, but more research is needed for their routine use. Additionally, the risk of NLN progressing to malignant lesions highlights the importance of vigilant monitoring and management. This study aims to analyze the relationship between microcalcifications and NLN, addressing progression risks and implications for clinical practice. Method: This systematic review and meta-analysis (CRD42022346891) involved a comprehensive search of databases such as PubMed and Scopus from 2000 to 2023, using keywords related to lobular carcinoma and mammography. Eligible English-language studies included those reporting mammographic findings of pure NLN lesions confirmed by histopathologic evaluation and surgical excision. Exclusion criteria involved studies without surgical results or definitive imaging findings. Two independent reviewers assessed titles and abstracts, resolving discrepancies as needed. Data were systematically extracted using a standardized form, with the selection process depicted in a PRISMA flow diagram. Result: Meta-analysis of included studies revealed that the pooled proportion of any mammographic microcalcification among all NLN lesions was 0.70 (95% CI: 0.63-0.76). Pure microcalcification (without an associated mass or distortion) was the most common presentation, with a pooled proportion of 0.67 (95% CI: 0.60-0.74) among all lesions and 0.99 (95% CI: 0.96-1.00) among lesions presenting with any microcalcification. The overall pooled upgrade rate to malignancy was 0.18 (95% CI: 0.11-0.25), with a significantly higher rate for lobular carcinoma in situ (LCIS) at 0.22 (95% CI: 0.15-0.30) compared to atypical lobular hyperplasia (ALH) at 0.06 (95% CI: 0.01-0.14). Microcalcifications were present in the majority of upgraded lesions (pooled proportion: 0.78, 95% CI: 0.69-0.87). A small but significant proportion of lesions (0.08, 95% CI: 0.03-0.17) had no mammographic findings. All pooled estimates showed high heterogeneity. Sensitivity analysis confirmed the robustness of the results, while Egger's test indicated potential publication bias. Conclusion: In conclusion this study highlights the significant prevalence of microcalcifications in NLN cases and their critical role as a diagnostic feature. Despite their association with disease progression, microcalcifications are not reliable predictors of malignancy upgrade. Further evaluation is necessary to understand their clinical implications and improve management strategies for patients with NLN.

چکیده

Background: The purpose of this study is to evaluate the performance and capability of ancillary features to upgrade LR-4 to LR-5 for the hepatocellular carcinoma diagnosis. Upgrading a liver lesion from LI-RADS category LR-4 (probably HCC) to LR-5 (definitely HCC) enhances diagnostic confidence by confirming a definitive noninvasive diagnosis of HCC. This upgrade facilitates prompt initiation of curative or locoregional treatments without the need for biopsy, aligns patients with eligibility criteria for liver transplantation under established guidelines, and supports timely inclusion in treatment planning pathways. Transitioning from LR-4 to LR-5 also shifts clinical management from surveillance to active treatment, ultimately aiming to improve patient outcomes. Main body of the abstract: Up to 2025, PubMed, Scopus, Web of Science, and Embase were searched comprehensively. All studies discussing the role of ancillary features in upgrading LI-RADS from 4 to 5 were included except review articles, articles published in a language other than English, and case reports or series of cases. A literature review and data extraction were performed by two independent reviewers. A checklist for diagnostic studies developed by The Quality Assessment of Diagnostic Accuracy Studies 2 (QUADAS-2) scale was used to assess sources of bias. This systematic review included seven studies. Conclusions: The current systematic review reveals the good performance of ancillary features in combination with major features for upgrading LR-4 lesions to LR-5 so that these features provide high sensitivity while maintaining acceptable specificity for diagnosing hepatocellular carcinoma. We strongly recommend the routine integration of ancillary features in LI-RADS assessments to improve diagnostic accuracy and guide appropriate management, which ultimately results in better patient's outcomes.

چکیده

Purpose: Radiologic examinations frequently identify cystic liver lesions, which encompass various entities from simple benign cysts to malignant neoplasms. This work analyses the available data to compare diagnostic features of biliary cystic neoplasms and hepatic simple cysts. Methods: A systematic search of PubMed, Scopus, Embase, and Web of Science up to October 2024 was conducted. The characteristics were categorized into hepatic simple cysts (HSC) and mucinous cystic neoplasms (MCN), including biliary cystadenoma (BCA) and cystadenocarcinoma (BCAC) detected by imaging modalities including ultrasound, CT scans with IV contrast, or MRI. We analyzed biliary cystic neoplasms and hepatic simple cysts across multiple studies using Review Manager Ver. 5, calculating summary measures for each feature. Results: The study analyzed 577 lesions in 577 patients and 49 studies. Hepatic simple cysts were the most common finding, with 349 identified, mainly in the right hepatic lobe, presented with abdominal pain or incidentally. Intracystic septation was found in 50.1% of HSC lesions, with thick septation in 10.52% of lesions. 228 (49.9%) patients were diagnosed with MCN, with abdominal swelling and pain as the most common presentation. Septation was the most common radiological feature of MCNs, with thick septa in 50.61%. MCNs had internal septa, solid mural nodule, upstream bile duct dilation, presence in the left hepatic lobe, septal thickening, cystic wall enhancement, calcifications, and internal debris. The presence of a cyst in the left lobe was more related to MCNs. Conclusion: Characterizing cystic liver lesions necessitates a comprehensive evaluation of the lesions' location, size, and complexity. Imaging and clinical findings are essential for a final diagnosis.

چکیده

Hepatocellular adenomas are rare benign liver tumors primarily affecting young women, particularly those using oral contraceptives. This systematic review and meta-analysis aimed to evaluate the radiologic features of beta-catenin mutated HCAs with a focus on enhancing diagnostic accuracy and informing management strategies. A comprehensive search of databases yielded 328 articles, with 13 meeting inclusion criteria. The analysis included 94 B-HCAs, revealing a mean size of 6.46 cm and a notable prevalence of necrosis (32.65%). Imaging characteristics demonstrated that T1-weighted MRI showed isointensity in 47% and hypointensity in 36% of lesions, while T2-weighted MRI indicated iso-mild hyperintensity in 60% and hyperintensity in 38%. In the arterial phase, 87% of lesions exhibited hyperenhancement. The presence of a central scar was noted in 47.05% of cases, while the atoll sign and intralesional hemorrhage were less common. The study found a significant tendency for malignant transformation, with 47.72% of B-HCAs evolving into malignant lesions. The findings suggest that B-HCAs may present features similar to focal nodular hyperplasia, complicating diagnosis. Given the high risk of malignancy, management strategies for B-HCAs should align with those for hepatocellular carcinoma. This review underscores the importance of comprehensive MRI evaluation and clinical context in diagnosing B-HCAs, advocating for cautious interpretation of imaging findings to guide potential biopsy decisions.

چکیده

Objective: To assess the efficacy of cryoablation in controlling pain from metastatic bone lesions. Materials and methods: A systematic search of PubMed, Embase, Scopus, Cochrane library, and Web of Science was conducted from inception to April 2024, focusing on cryoablation for palliation of painful bone metastases. The inclusion criteria were as follows: studies involving patients over 18 years of age who were affected by bone metastases; bone metastases treated with stand-alone cryoablation; studies reporting patients' pain levels before and at least at one time point after cryoablation; and studies published in English. Results: A total of 844 articles were initially screened, resulting in 12 articles involving 309 patients included. Pain assessments were conducted at various time points ranging from 1 day to 6 months after the cryoablation procedure. Included studies reported significant improvements in pain scores based on the visual analog scale (VAS), the numeric rating scale (NRS) and brief pain inventory-short form (BPI-SF) following treatment at 1, 4, 8 and 12 weeks. The most notable mean difference between pre- and post-procedure pain scores was observed at 12 weeks, with a standardized mean difference of -3.71 (95% confidence interval [CI]: -5.29 to -2.00; p < 0.001). Regarding pain relief outcomes, by the fourth week, the proportion of patients experiencing pain relief was 0.69 (95% CI: 0.62 to 0.75; p < 0.001). Conclusions: Cryoablation could be an effective method for palliation of painful bone metastases. Further studies are needed to compare its efficacy with other palliative methods and to define its role in cancer management.

چکیده

Purpose: The Ki-67 marker reflects tumor proliferation and correlates with meningioma prognosis. Here we aim to evaluate the performance of MRI-derived radiomics for Ki-67 index prediction in meningiomas. Methods: After a comprehensive search in Web of Science, PubMed, Embase, and Scopus, data extraction and risk of bias assessment was performed. Pooled sensitivity, specificity, positive likelihood ratios (PLR), negative likelihood ratios (NLR), and diagnostic odds ratio (DOR) were computed. The summary receiver operating characteristic (sROC) curve was generated and area under the curve (AUC) was calculated. Separate meta-analyses were conducted for radiomics models and combined models. Heterogeneity was evaluated using the I2 statistic, and subgroup analysis was performed to identify potential sources of heterogeneity. Sensitivity analysis was carried out to detect possible outliers. Results: Seven studies were included, with six studies analyzed for radiomics model and four for combined model. For radiomics model, the pooled sensitivity, specificity, PLR, NLR, DOR, and AUC were 67%, 82%, 8.61, 3.54, 0.43, and 0.79, respectively. For combined model, pooled sensitivity, specificity, PLR, NLR, DOR, and AUC were 78%, 78%, 12.19, 3.47, 0.30, and 0.79, respectively. Sensitivity analysis identified no outliers. In radiomics model, potential sources of heterogeneity included mean age and the application of N4ITK bias correction. For combined model, heterogeneity was influenced by mean age, application of N4ITK bias correction, and the use of external validation. Conclusion: Radiomics shows promising ability to predict the Ki-67 index status in meningioma patients, potentially enhancing clinical decision-making and management strategies.

چکیده

Background: Approximately 20% of all transient ischaemic attacks (TIAs) and ischaemic strokes occur within the posterior circulation, with vertebrobasilar stenosis identified as the cause in roughly 25% of the cases. Studies have shown that about a quarter of these patients have atherosclerotic stenosis of at least 50% of the vertebrobasilar artery. Stenosis has been shown to be associated with an increased risk of 90-day recurrent vertebrobasilar stroke, particularly in the first few weeks, which is significantly higher when compared with patients with stenosis of the anterior circulation. Therefore, aggressive treatment is important for the patient's prognosis. Stenting is emerging as a promising therapeutic strategy for persistent ischaemia events that do not respond to the best medical treatment, but it is not without complications. We systematically reviewed the literature on percutaneous transluminal angioplasty and stenting (PTAS) for intracranial vertebrobasilar artery stenosis (IVBS). Methods: PubMed, Web-of-Science and Scopus were searched upon the Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines to include prospective/retrospective cohort, randomised/non-randomised clinical trials and case series studies describing PTAS for IVBS. Pooled rates of intervention-related complications and outcomes were analysed with random-effect model meta-analysis using StataMP V.18.0 software. Results: 31 studies were found eligible which included 1928 cases. 1103 basilar artery stenosis cases were reported in 27 studies 0.65 (95% CI 0.53, 0.76), I2: 99.72%. 648 vertebral cases were reported in 18 studies 0.60 (95% CI 0.49, 0.70), I2: 97.49%. In four studies, the rate of vertebrobasilar stenosis cases calculated as a proportion of the total sample size was 0.10 (95% CI 0.05, 0.15). Mean stenosis in 21 included studies was found to be 0.83 (95% CI 0.79, 0.88), I2: 0.00%, which shows variation of baseline stenosis between studies was minimal. 51 deaths were recorded in 24 studies. Meta-analysis of mortality showed the overall rate of mortality was 0.03 (95% CI 0.02, 0.05), I2: 44.90%. In 14 studies, symptomatic intracranial haemorrhage events were recorded at an overall rate of 0.01 (95% CI 0.00, 0.02), I2: 0.00%. Generally, a follow-up period of at least 3 months was reported in the included studies. Furthermore, procedural stroke/TIA was evaluated in seven studies, four of which reported no events (0.03 (95% CI 0.00, 0.08), I2: 20.38%). Mean time from initial symptoms to recanalisation was 23.98 (95% CI 18.56, 29.40), I2=98.8%, p=0.00 days. Conclusion: In certain individuals with medically unresolved, severe, symptomatic and non-acute IVBS, elective vertebrobasilar PTAS appears to be both safe and effective. Various stent designs and angioplasty-assisted techniques should be taken into consideration based on the specific clinical and radiological traits of the lesions. Future randomised controlled trials are required to verify these results.

چکیده

[Assembled from the ScienceDirect record sections and article highlights; full structured abstract not retrieved in one block] Background: Congenital heart disease is the main cause of perinatal morbidity and mortality. Aim: To evaluate the pooled diagnostic test accuracy of first-trimester screening markers (nuchal translucency, ductus venosus and tricuspid regurgitation) for congenital heart defects. Key concepts: Systematic review and meta-analysis of PubMed, Scopus, Web of Science and Embase (search until 8 March 2024; PROSPERO CRD42023401007), using a bivariate random-effects summary receiver operating characteristic model; 42 studies, 745,580 participants, 2,755 with congenital heart disease. For major congenital heart disease, pooled sensitivity and specificity were: A/R ductus venosus a wave 43.1% and 95.5%; abnormal ductus venosus pulsatility index for veins 57.8% and 88.8%; tricuspid regurgitation 37.0% and 97.7%; nuchal translucency above the 95th percentile 41.4% and 93.7%; nuchal translucency above the 99th percentile 26.6% and 98.3%. The most sensitive combination (nuchal translucency above the 95th percentile, or A/R ductus venosus a wave, or tricuspid regurgitation) reached 61.4% sensitivity, and the highest specificity (nuchal translucency above the 95th percentile plus A/R ductus venosus a wave) reached 97.8%. The authors conclude that first-trimester nuchal translucency, ductus venosus and tricuspid regurgitation assessment is a useful adjunct for congenital heart disease screening, with combinations of markers improving detection.

Methods: Medline via PubMed, Scopus, Web of Science, ProQuest, and Embase databases were systematically searched from January 2013 to July 2022 for original studies, written in English, reporting the positive predictive value (PPV) of individual BI-RADS 5th edition descriptors (distribution and type of enhancement) of NME lesions. Risk of bias and quality of included studies were assessed by QUADAS 2 appraisal tool. Odds ratio (OR) of pathologically confirmed malignant results in each distribution and internal enhancement were pooled in a meta-analysis. Results: Eight studies for a total of 1095 lesions were included. The pooled OR of malignancy for linear, focal, segmental, regional, multiple region, and diffuse distributions are 0.70 (95%CI: 0.44-1.14), 0.37 (95% CI: 0.26-0.54), 2.42 (95% CI: 1.62-3.62), 0.56 (95% CI: 0.11-2.79), 2.80 (95% CI: 0.96-8.21), and 3.35 (95% CI: 0.59-19.04), respectively. The pooled OR of malignancy for homogenous, heterogeneous, clustered ring enhancement, and clumped enhancement are 0.39 (95% CI: 0.23-0.67), 0.59 (95% CI: 0.40-0.85), 2.92 (95% CI: 1.86-4.57), and 1.49 (95% CI: 0.96-2.32), respectively. Conclusion: Based on a meta-analysis of 8 studies and more than one thousand non-mass enhancing lesions, diffuse, multiple regions and clustered ring descriptors of enhancement have the highest pooled OR for malignancy.

چکیده

Purpose: Splenic artery embolization (SAE) has emerged as a promising alternative for managing variceal bleeding secondary to portal hypertension (PH). This study aims to elucidate the significance of SAE in managing esophageal variceal bleeding in patients with PH, providing an overview of its efficacy, safety, and role in PH management. Methods: A systematic review and meta-analysis were conducted in accordance with PRISMA standards. EMBASE, PubMed, Scopus, and Web of Science databases were searched from inception until April 14, 2024. Original observational and clinical studies on SAE in managing variceal bleeding due to PH were included. Meta-analyses were performed using a random-effects model, and publication bias was assessed using regression and rank correlation tests for funnel plot asymmetry. Results: Eighteen studies met the inclusion criteria, encompassing 531 patients. The meta-analysis revealed a significant reduction in variceal bleeding post-SAE (RD = -0.86; 95% CI: -0.97, -0.75; p < 0.001). Complete resolution of varices was observed in 26% of patients (95% CI: 11%, 45%; p = 0.006), and 78% showed improvement in variceal grade (95% CI: 43%, 88%; p < 0.001). SAE significantly increased platelet counts (SMD = 1.15; 95% CI: 0.63, 1.68; p < 0.001). Common complications included post-embolization syndrome, and the overall complication rate was low. Conclusions: This systematic review and meta-analysis study supports the efficacy and safety of SAE in managing variceal bleeding due to PH, demonstrating significant reductions in bleeding, improvements in variceal grade, and increases in platelet counts.

چکیده

Background: Low-dose aspirin (ASA) is used in obstetrics for different indications, mainly to prevent preeclampsia. This study investigated the underlying mechanism of ASA's effect on the fetus's cardiovascular functions. Methods: 42 pregnant women at 18-24 weeks of gestation, identified as high-risk for preeclampsia, received 160 mg of ASA daily. Fetal Doppler ultrasound was performed before and three weeks after ASA treatment, assessing ductus venosus, middle cerebral, umbilical, and uterine arteries pulsatility indices as well as pulmonary, aorta, and superior vena cava (SVC) diameters in the three-vessel view, including pulmonary/aorta and SVC/aorta ratios. All analyses were performed using SPSS software version 27, with a significance threshold set at p < 0.05. A paired t-test was used to assess differences in means. The Chi-square and Fisher's exact test analyzed nominal variables. Results: Post-intervention analysis revealed significant improvements in abnormal uterine artery resistance (p < 0.001) and abnormal pulsatility index of the umbilical artery, middle cerebral artery, and ductus venosus (p < 0.001 for all). Moreover, 160 mg/day aspirin administration significantly increased mitral E/A (early filling velocity/atrial contraction velocity: 0.397 ± 0.029; p < 0.001), diameters of aorta (4.390 ± 0.852; p < 0.001), pulmonary artery (4.895 ± 1.087; p < 0.001), and SVC (2.511 ± 0.535; p < 0.001), while significantly decreasing left ventricular myocardial performance index (p < 0.05). Conclusion: Daily administration of 160 mg of aspirin enhances fetal vascular and cardiac function. Evaluating fetal cardiovascular parameters beyond routine uterine artery Doppler, especially in high-risk pregnancies, and initiating ASA therapy in cases of insidious abnormalities, may help delay or prevent fetal complications such as intrauterine growth restriction (IUGR) by improving cardiovascular function.

چکیده

Background: Approximately 8% of breast cancers originate within the central ducts near the nipple-areolar complex (NAC), which has revealed higher rates of occult involvement than previously thought. Precise preoperative imaging to assess NAC and subjacent tumor involvement has become critical to identify candidates for nipple-sparing mastectomy (NSM) while ensuring oncological safety. The aim of our study is to determine whether Magnetic Resonance Imaging (MRI) could be used for the detection of subtle neoplastic infiltration. Method: Electronic databases including PubMed, Scopus, Web of Science, and EMBASE were systematically searched to identify all relevant studies published before 2024 on diagnostic performance of MRI in malignant extension to the nipple. The keywords included MRI, NAC, breast cancer, NSM, True Positive (TP), True Negative (TN), False Positive (FP), False Negative (FN) were extracted for analysis. Finally, 5 articles were selected for our meta-analysis. STATA version 15 was used to analysis data. Result: Our meta-analysis showed a pooled sensitivity of 85% (95% CI: 77%-91%) with low heterogeneity (I2 = 00.00%), and a pooled specificity of 82% (95% CI: 49%-95%), with significant heterogeneity (I2 = 98%). The pooled positive diagnostic likelihood ratio (pDLR) was 4.6 (95% CI: 1.3-16.6, I2 = 96.66%), and the negative diagnostic likelihood ratio (nDLR) was 0.18 (95% CI: 0.10-0.33, I2 = 50.06%). Conclusion: Diagnostic MRI characteristics such as NAC enhancement, non-mass enhancement (NME) type, mass size (greater than 20 mm), and tumor-to-nipple distance (TND) were associated with the presence of NAC involvement. Because of high diagnostic accuracy, MRI imaging can be used as a helpful tool for NAC diagnosis.

چکیده

Various soft markers can be detected in the ultrasonography of foetuses, which can be related to chromosomal abnormalities and increases the risk of abnormalities, or they can be considered as normal variations that can disappear due to the pregnancy progress. There are different tools to detect chromosomal abnormalities like conventional karyotyping, chromosomal microarray analysis (CMA), single nucleotide polymorphism (SNP) array, non-invasive prenatal test (NIPT), and non-invasive prenatal screening (NIPS). Therefore, in the present study, we aim to assess the accuracy of ultrasonic soft markers in the diagnosis of chromosomal abnormalities such as chromosomal structural abnormalities, aneuploidy, and triploidy, especially Trisomy 21 and Trisomy 18. A systemic literature search was performed using PubMed, Scopus, Google Scholar, and Web of Science. We gathered all articles published before August 2023. We selected English studies such as retrospective and cross-sectional ones that assessed the relationship between ultrasonic soft markers and foetal chromosomal abnormalities. A total of 10 articles with 18,580 cases were included in our systematic review article that assessed the foetal abnormalities and aneuploidies by using conventional karyotyping, SNP array, CMA, and NIPT (or NIPS). Trisomy 21, Trisomy 18, and chromosomal structural abnormalities were the most common abnormalities related to ultrasonic soft markers by karyotyping; however, Trisomy 13, 47, XXY, 45, X, and mosaic chromosomal abnormalities were other abnormalities detected. Results by CMA showed Trisomy 21 and Trisomy 18 as the most common abnormalities in the foetuses also with ultrasonic soft markers, and other abnormalities were pathogenic copy-number variations, Turner (XO), polyploidy, 22q11.2 deletion, and Trisomy 13, respectively. The authors conclude that soft markers should be employed besides CMA, SNP array, and NIPS/NIPT.

چکیده

Background and objectives: The spleen hosts both benign and malignant lesions. Despite multiple imaging modalities, the distinction between these lesions poses a diagnostic challenge, marked by varying diagnostic accuracy levels across methods. In this study, we aimed to evaluate and compare the diagnostic performance of various imaging techniques for detecting malignant splenic lesions. Methods: Following PRISMA guidelines, we searched PubMed, Scopus, and Web of Sciences databases for studies evaluating imaging techniques in detecting malignant splenic lesions. Data extraction included diagnostic accuracy metrics, and methodological quality was assessed using QUADAS-2. Diagnostic Test Accuracy meta-analyses were conducted using R (version: 4.2.1). Subgroup analyses and meta-regression were performed to compare different modalities and clinical settings. Results: Our study included 28 studies (pooled sample size: 2358), primarily using retrospective designs with histopathology as the reference standard. PET scan demonstrated the highest diagnostic accuracy (AUC: 92%), demonstrating a sensitivity of 93% (95% CI: 80.4%-97.7%) and a specificity of 82.8% (95% CI: 71.1%-90.4%). Contrast-enhanced ultrasound (CEUS), Contrast-enhanced CT scan, and contrast-enhanced MRI also showed impressive performance with AUCs of 91.4%, 90.9%, and 85.3%, respectively. Differences among these modalities were not statistically significant, but they outperformed non-contrast-enhanced methods. PET and CEUS exhibited higher specificity for lymphoma cases compared to studies including other malignancies. Conclusion and clinical implications: Overall, PET emerges as the best modality for splenic malignancies, and CEUS and CE-MRI show promise as potential alternatives, notably due to their reduced radiation exposure. Further research is essential for precise malignancy differentiation.

چکیده

Objectives: Splenic lesions might exhibit overlapping imaging features, varying from benign entities like cysts and hemangiomas to malignancies such as lymphoma and angiosarcoma. This meta-analysis aims to delineate imaging characteristics that distinguish malignant from benign splenic lesions. Methods: Adhering to PRISMA guidelines, we searched PubMed, Scopus, and Web of Science for studies on imaging features differentiating malignant from benign splenic lesions. We extracted data on splenic pathology and imaging characteristics and assessed the methodological quality via QUADAS-2. Odds ratio meta-analyses were performed using STATA (Version 17.0, Stata Corp, College Station, TX). Results: Portal phase hypoenhancement, hypovascular enhancement pattern, diffusion restriction, and late phase hypoenhancement, with odds ratios above 10, highly indicate malignancy. Other features suggestive of malignancy include solid morphology, lymphadenopathy, presence of perisplenic fluid, arterial hypoenhancement, hypoechogenicity on ultrasound, splenomegaly, and presence of multiple lesions. In contrast, cystic morphology, hypervascular-washout and hypervascular-persistent pattern of enhancement, late phase hyperenhancement, anechogenicity on ultrasound, portal phase hyperenhancement, well-defined borders, and calcification are in favour of benign pathology. Conclusion: The study underscores the critical role of contrast-enhanced and diffusion-weighted imaging in distinguishing malignant from benign splenic lesions, emphasizing the role of features like portal phase hypoenhancement and restricted diffusion in diagnosing malignancies. Additionally, the study emphasizes the value of contrast-enhanced ultrasound, which allows for the visualization of key contrast-enhancement patterns without the risk of ionizing radiation exposure.

Abdominal Radiology · 2024 · استناد در گوگل اسکولار: 1

چکیده

Gynecological malignancies, such as ovarian cancers, cervical cancers, and endometrial cancers, have a significant global impact. Women with gynecologic malignancies may receive a single or a combination of treatments, including surgery, chemotherapy, and radiation-based therapies. Radiologists utilize various diagnostic imaging modalities to provide the surgeon with relevant information about the diagnosis, prognosis, optimal surgical strategy, and prospective post-treatment imaging. Computerized Tomography (CT) and magnetic resonance imaging (MRI) may be used initially to evaluate and detect post-treatment complications. Although CT is primarily used for staging, MRI is commonly used for a more accurate evaluation of a tumor's size and detection of local invasion. Complications such as hematoma, abscess, inclusion cyst, seroma, tumor thrombosis, anorectovaginal fistula, and gossypiboma may occur after the three primary treatments, and systems such as the genitourinary, gastrointestinal, neurological, and musculoskeletal may be affected. In order to distinguish between early-onset and late-onset complications following gynecological treatment, radiological findings of the most common post-treatment complications will be presented in this review.

چکیده

Ectopic varices account for 5% of variceal bleedings and occur outside the gastro-esophageal region. This review evaluates the efficacy of transjugular intrahepatic portosystemic shunt (TIPS) for ectopic variceal management. A comprehensive search through PubMed, Scopus, Web of Science, and Embase was conducted until January 16, 2023, using relevant keywords. Case reports and case series with fewer than 10 patients on TIPS for ectopic variceal management were included. The quality assessment followed the Joanna Briggs Institute checklist for case reports. This systematic review evaluated 43 studies involving 50 patients with ectopic varices undergoing TIPS. Patients had a mean age of 54.3 years, half were female, and two were pregnant. Alcoholic liver disease (48%) and hepatitis C infection (26%) were common causes of portal hypertension. Ascites and splenomegaly were reported in 32% and 28% of the patients, respectively. Rectal, oral, and stomal variceal bleeding accounted for 62%, 16%, and 22% of the patients, respectively. Ectopic varices were mainly located in the duodenum (28%) and rectum (26%) regions. Complications affected 42% of the patients, re-bleeding in eleven and hepatic encephalopathy in seven. The follow-up lasted 12 months on average, and finally, 5 received a liver transplant. Mortality post-TIPS was 18%. Despite complications and a notable mortality rate, favorable outcomes were observed in almost half of the patients with ectopic variceal bleeding managed with TIPS. Further research is warranted to refine strategies and improve patient outcomes.

چکیده

Background: Developmental dysplasia of the hip (DDH) is used to describe a spectrum of structural abnormalities that involve the growing hip. Early diagnosis and treatment are critical to providing the best possible functional outcome. This study aimed to evaluate the prevalence of DDH in neonates with and without risk factors and determine the role of ultrasound screening on the initial diagnosis. Methods: This prospective cross-sectional study was conducted on 399 infants at the Pediatric Treatment Center, Tehran University of Medical Sciences, between December 2015 and June 2016. Infants with suspected DDH who underwent hip ultrasonography were included, and the presence or absence of each risk factor was documented according to the checklist. The ultrasound findings were also registered in the checklists. The odds ratio (OR) of each risk factor for DDH was calculated. The collected data were analyzed by SPSS software version 18 at a 0.05 significance level. Results: In 16 months of study, 174 (43.6%) male and 225 (56.4%) female infants under the age of 18 months were studied. Risk factors were detected in the medical history of 329 infants. Out of them, 230 (57.6%) were firstborn children, 7 (1.75%) had a positive family history of DDH, and 26 (6.5%) had limb anomalies. There was also a history of breech presentation in 16 (4.01%) and a history of oligohydramnios in 21 (5.1%) of infants. The prevalence of DDH was 25.8% in infants with risk factors and 2.8% in those without risk factors (OR = 11.84, P < 0.05). Conclusion: In this study, the frequency of DDH was significantly higher in infants with risk factors. The female gender and limb anomalies were stronger risk factors for DDH. Overall, ultrasound showed great potential for DDH screening.

Research Square (preprint) · 2024

چکیده

Background: MRI is pivotal in breast imaging, encompassing staging, treatment monitoring, and lesion differentiation. While MRI boasts high sensitivity, specificity, and utility in detecting otherwise unseen lesions, challenges persist in accurately distinguishing benign from malignant findings. The study delves into MRI-guided breast biopsy outcomes and highlights the importance of radiologic-pathologic results. Methods: This retrospective study analyzed 109 MRI-guided breast biopsies conducted on lesions identified between 2017 and 2023. The patients underwent biopsies for screening and diagnostic purposes. Biopsy procedures involved meticulous MRI guidance using a 1.5 Tesla system. Lesions were categorized based on location and BI-RADS lexicon, with biopsy results spanning benign, suspicious, and malignant pathologies. Data collection encompassed a wide array of patient factors and pathology reports, meticulously reviewed by experienced radiologists, shedding light on the efficacy and outcomes of MRI-guided breast biopsies. Results: The participants had a mean age of 45 ± 11 years. A significant association was found between the history of pregnancy and breast lesion enhancement. Patients with mass enhancement had a higher BI-RADS B4b, B4c, and B5 classification rate, while those with non-mass enhancement were more commonly classified as BI-RADS B3 and B4a. Histopathology diagnoses were significant in determining the presence of mass or non-mass lesions. The sensitivity and specificity of MRI for detecting malignancy were high for BI-RADS categories 4c and 5 but may result in a higher number of false positives. Conclusions: Our research highlighted the significance of MRI in the diagnosis of breast cancer, particularly when used in conjunction with high-risk lesions as well as showed the need of sub-classifying BI-RADS-4 lesions to minimize the number of unnecessary biopsies. The results affirm the ongoing use of MRI-guided biopsy for the detection of breast cancer. Note: the preprint PDF spells one co-author's surname "Golazar" (M. H. Golazar).

Turkish Journal of Immunology · 2024

چکیده

Infectious diseases are among the most important global health problems, and their burden on society is wide. It is shown that chimeric antigen receptor (CAR) T-cell therapy can fight viral infectious diseases in many aspects. This systematic review investigated the efficacy of CAR T-cell therapy against viral infections. A comprehensive literature search was conducted in Medline, Scopus, Cochrane Central Library, and Embase up to May 2024. Keywords such as CAR T-cell therapy, viral, virus, and infection were used to identify relevant studies. In the identification stage, a comprehensive search across databases, including Medline, Scopus, Cochrane Central Library, and Embase, identified 12183 papers. Finally, 7 articles with total of 134 individuals were examined. Three studies investigated the efficacy of CAR T-cell therapy against human immunodeficiency virus (HIV), hepatitis B virus (HBV), and cytomegalovirus (CMV). The primary objectives of these trials were to assess the safety and feasibility of adoptive transfer of CAR T-cells in patients with viral infections. The aforementioned results demonstrate the efficacy of CAR T-cell therapy in targeting viral antigens and inhibiting viral replication, providing possible treatment options for long-term infectious illnesses like HIV, HBV, and CMV. These findings show the efficacy of CAR T-cell therapy in infectious diseases, but the data are limited. To maximize the efficacy of CAR T-cell therapy and clarify its long-term effectiveness in the treatment of infectious diseases, more investigation and clinical studies are required.

چکیده

Atlantoaxial rotatory fixation (AARF) in adults is a rare and clinically challenging condition characterized by a spectrum of etiological factors, predominantly attributed to traumatic and inflammatory pathologies within the craniovertebral region. Trauma is the most frequently identified cause within the adult population, with the first case report published in 1907. This study aims to conduct a systematic review that addresses the clinical presentations and management strategies relating to traumatic atlantoaxial rotatory fixation in adults. A comprehensive search of the PubMed database was executed, adhering to the PRISMA guidelines. The inclusion criteria encompassed case reports and series documenting AARF cases in individuals aged 18 and above, spanning database inception to July 2022. Studies not published in the English language were excluded. A total of 61 articles reporting cases of AARF in the adult population were included in the study. The mean age of affected individuals was 36.1 years (± 15.6), with a distribution of 46% females and 54% males. Predominant mechanisms of injury included motor vehicle accidents and falls, constituting 38% and 22% of cases, respectively. Among the classification systems employed, Fielding and Hawkins type I accounted for the majority at 63%, followed by type II at 10%, and type III at 4%. Conservative management was used for treatment in 65% of acute cases and 29% of chronic cases. Traumatic AARF is a rare phenomenon in the adult population, is more common in younger adults, and does not often present with neurologic deficits. Patients diagnosed acutely are more likely to be successfully treated with conservative management, while patients diagnosed chronically are less likely to be reduced conservatively and often require surgical treatment. Surgery should be considered for patients with irreducible dislocations, ligamentous injuries, unstable associated fractures, and persistent pain resistant to conservative management.

چکیده

Purpose: Magnetic resonance imaging (MRI) can reduce the need for unnecessary invasive diagnostic tests by nearly half. In this meta-analysis, we investigated the diagnostic accuracy of intravoxel incoherent motion modeling (IVIM) and dynamic contrast-enhanced (DCE) MRI in differentiating benign from malignant breast lesions. Method: We systematically searched PubMed, EMBASE, and Scopus. We included English articles reporting diagnostic accuracy for both sequences in differentiating benign from malignant breast lesions. Articles were assessed by quality assessment of diagnostic accuracy studies-2 (QUADAS-2) questionnaire. We used a bivariate effects model for standardized mean difference (SMD) analysis and diagnostic test accuracy analysis. Results: Ten studies with 537 patients and 707 (435 malignant and 272 benign) lesions were included. The D, f, Ktrans, and Kep mean values significantly differ between benign and malignant lesions. The pooled sensitivity (95% confidence interval) and specificity were 86.2% (77.9%-91.7%) and 70.3% (56.5%-81.1%) for IVIM, and 93.8% (85.3%-97.5%) and 68.1% (52.7%-80.4%) for DCE, respectively. Combined IVIM and DCE depicted the highest area under the curve of 0.94, with a sensitivity and specificity of 91.8% (82.8%-96.3%) and 87.6% (73.8%-94.7%), respectively. Conclusions: Combined IVIM and DCE had the highest diagnostic accuracy, and multiparametric MRI may help reduce unnecessary benign breast biopsy.

چکیده

Background: Endometriosis is a benign gynecologic condition that is estrogen-dependent. Malignant transformation is one of the complications of endometriosis; it can affect the woman's ovarian and extra-ovarian regions. Diagnosing endometriosis-associated cancers has an essential role in treatment. Purpose: Our study aimed to review magnetic resonance imaging (MRI) features of malignant transformation of pelvic endometriosis. Material and Methods: We searched three online databases including Web of Science, Scopus, and PubMed, applying relevant keywords up to May 2022. We included English observational studies using MRI for endometriosis patients and comparing the MRI features of malignant pelvic endometriosis with non-malignant ones or studies reporting the MRI features of malignant pelvic endometriosis. Finally, a comprehensive systematic review of the included studies was conducted. The small number of qualifying studies was the most significant limitation. Also, the eligible studies were insufficient for conducting a meta-analysis. Results: A total of eight papers were included as they matched our inclusion requirements. Eighty-seven individuals were included finally. Studies were published between 1992 and 2021. The larger size of the lesion, loss of T2-weighted images shading, mural nodules, mural nodules contrast enhancement, and mural nodules protruding from the cyst wall at acute angles were the main MRI features suggesting potential malignant transformation of pelvic endometriosis. Conclusion: The endometriosis's malignant changes can be observed on MRI. The predominant MRI findings are a large cyst, loss of shading on T2-weighted imaging, mural nodules enhancement, and acutely angled protrusion of mural nodules from the cyst wall.
